Air combat reports

In this, the centennial year of The Fokker Scourge, etc, I got to wondering (WATCHOUT!) when the various air arms began issuing air combat report forms. I believe the RFC (don't know about RNAS) started sometime in 1915 and assume the French & Germans were approximately similar. Anybody know dates of issue?

Related question: what was the format for aerial encounters before the advent of "Combats in the Air," etc?
